Ciliary cysts as uncommon manifestation of intraocular toxocariasis

Purpose

To describe an atypical clinical presentation of Toxocara canis intraocular affection.

Setting/Venue

A 72–year-old male patient, without remarkable clinical history, presented unilateral anterior chamber inflammation, middle vitritis and cystoid macular edema. Furthermore, a peripheral pre-retinal neovascular network was shown, surrounded by an ischemic area.

Methods

A complete conventional uveitis work-out was performed without remarkable findings, except from the presence of ciliar pseudocysts in ultrasonic biomicroscopy (UBM), which leaded to the presumption diagnosis of ocular toxocariasis. It was confirmed by serology and aqueous PCR.

Results

Pseudocyts, although unfrequented, have been already described to be presented in ocular toxocariasis. UBM technique is critical to reach this diagnose. Choroidal neovascularization has been mentioned between its clinical features, however, it is the first time in bibliography that a pre-retinal vascular network is described related to this pathology. Aqueous or vitreous sampling for eosinophilia, antibody detection and PCR could be necessary if the serological tests are not conclusive.

Conlusions

Ciliary cysts could be one of the manifestations of intraocular toxocariasis and they are relevant especially when the other typical features are not presented. Inflammation could lead also to a peripheral pre-retinal neovascular network that has not been described before.
