To describe the multimodal imaging findings in sclerochoroidal calcifications
Department of Ophthalmology, Fattouma Bourguiba University Hospital, Monastir, Tunisia.
Observational case reports
Case 1: A 79-year-old woman with a previous history of rheumatoid arthritis was referred for ophthalmologic screening. Her best corrected visual acuity (BCVA) was 20/20 in both eyes. Results of slit-lamp examination were unremarkable in both eyes. Fundus examination of the right eye showed multifocal pattern of small yellow–white subretinal lesions along the superotemporal retinal vascular arcades. Enhanced-Depth OCT showed elevated scleral masses with a rolling surface outpouching the RPE–choriocapillaris complex with outer retinal layer disturbance and reduction of choroidal vascular thickness from the base to the apex of the lesions. Fluorescein angiography (FA) showed early hyperfluorescent spots. Indocyanine green angiography (ICGA) demonstrated early hypofluorescent lesions that decreased in the late phases. B-scan ultrasonography showed hyperechogenic lesions with acoustic shadowing. Case 2: A 74-year-old patient presented with blurred vision of the right eye. He was single eyed and pseudophakic. His BCVA was 20/200. Results of slit-lamp examination were unremarkable. Fundus examination revealed an extensive geographic yellow–whitish lesion in the posterior pole. Swept source OCT showed an elevated scleral mass. Fundus autofluorescence showed hyperautofluorescence in the area of the lesion. FA demonstrated an area of early hyperfluorescence that increased in the late phases. B-scan ultrasonography demonstrated hyperechogenic lesion with acoustic shadowing.
In both cases, multimodal imaging findings were consistent with the diagnosis of sclerochoroidal calcifications and laboratory testings were done to rule out any disorders affecting calcium metabolism. Multimodal imaging provides useful information for the diagnosis and follow up of this condition.
