Multimodal Imaging of the Coexistence of Branch Retinal Vein Occlusion with Circumscribed Choroidal Hemangioma

Purpose

To describe the multimodal imaging findings in a case of branch retinal vein occlusion with coexisting circumscribed choroidal hemangioma

Results

A 50 year-old woman presented to our clinic with the complaint of decreased vision in her left eye. Her past medical history revealed hypertension and chronic renal failure. Vision was 20/25 in the right, counting fingers from 2 meters at the left eye. Fundus examination of the left eye revealed an orange-red elevated lesion just beneath the superior border of optic disc with increased vascular tortuosity at the superotemporal retina and macular edema. Optical coherence tomography imaging demonstrated large cystoid cavities and macular thickening. She was planned for MRI and simultaneous fluorescein and indocyanine green angiography (FA and ICG). The MRI imaging was unremarkable except for partial empty sella. FA and ICG imaging revealed a circumscribed choroidal hemangioma with superior retinal vein branch occlusion and secondary macular edema. The patient was initially treated with three loading doses of bevacizumab due to reimbursement regulations. After three doses visual acuity was improved to 20/40 and macular edema was resolved.

Conlusions

Coexistence of branch retinal vein occlusion and circumscribed choroidal hemangioma is a rare condition. Multimodal imaging may play an essential role in the diagnosis of that entities and monitoring treatment response.
