Purpose
To present ocular decompression retinopathy developing on the first postoperative day in a case where we performed clear lens extraction without complications due to angle-closure glaucoma.
Setting/Venue
This study conducted in Kütahya Health Sciences University Faculty of Medicine is a case report.
Methods
Clear lens extraction was planned for a 47-year-old female patient referred with the diagnosis of angle closure glaucoma. Intraocular pressure (IOP) was 50 mmHg. The patient was given triple antiglaucomatous drops, oral acetazolamide 3x1 and intravenous mannitol (1 mg / kg) preoperatively. Right eye clear lens extraction was completed without complications under topical anesthesia and eyelid akinesia.
Results
On the first postoperative day, the patient's right visual acuity (VA) was 0.4 and IOP 29 mm Hg. Fundus examination revealed diffuse blot hemorrhages around the optic disc and retina. Blocked fluorescence corresponding to blot hemorrhages were seen on fluorescein angiography. Our case was diagnosed with ocular decompression retinopathy. After the operation, triple antiglaucomatous treatment was continued and the number of antiglaucomatous was reduced to one due to the stable IOP in the follow-up. The retinal bleeding of the patient, whose right VA was 1.0 and IOP was 18 mm Hg, regressed considerably in the sixth week.
Conlusions
We report a ocular decompression retinopathy after uncomplicated clear lens extraction in a patient with narrow-angle glaucoma. It has been defined as a retinal complication frequently seen after glaucoma surgeries and complicated cataract surgeries in the literature.
