A single case report

Results

A 27-year-old male presented with bilateral blurred vision. He had a history of acquired immunodepression of unknown etiology associated with pneumocystis pneumonitis treated by cotrimoxazole. The fundus examination revealed multifocal bilateral yellowish round to oval multilobulated choroidal lesions associated with macular folds in the right eye. No vitritis was noted. The yellowish lesions corresponded to an early hypofluorescence and to late staining on fluorescein angiography. The macular folds were not visible on FA. Cotrimoxazole dosage was increased but the patient died days later.

Conlusions

Pneumocystis choroiditis is caused by Pneumocystis jiroveci previously known as Pneumocystis carinii. It is indicative of an associated disseminated pneumocystosis. Hence, despite not being an immediate vision threat, it is usually considered a life-threatening condition.
