Reticular-dystrophy-related choroidal neovascularization

Purpose

To report a case of choroidal neovascularization related to a reticular dystrophy

Methods

A single case report documented with multimodal imaging

Results

A 65-year-old male presented with metamorphopsia in his left eye since ten days. At presentation, he had a best-corrected visual acuity of 10/10 in the right and 8/10 in the left eye. Fundus examination found pigment clumping in both eyes associated to a retinal hemorrhage in the left eye. Autofluorescence showed a “fishing net-like” pattern of hyper-autofluorescence. Swept-Source-Optical coherence tomography (SS-OCT) findings included hyperreflective deposits between the retinal pigment epithelium (RPE)/Bruch’s complex and the interface of the IS/OS of the photoreceptors in both eyes associated to a serous macular detachment and to a flat irregular RPE detachment in the left eye. Optical Coherence Tomography Angiography (OCT-A) revealed choroidal neovascularization in the left eye. Fluorescein angiography showed a reticular pattern of hypo-fluorescence in both eyes with a heterogenous localized hyper-fluorescence in the left eye. These findings were consistent with the diagnosis of choroidal neovascularization related to Sjogren’s reticular pattern dystrophy. The patient underwent treatment with anti-VEGF injections in the left eye.

Conlusions

Sjogren reticular dystrophy is one of the pattern dystrophies which are a group of disorders characterized by a variable distribution of pigment deposition. Pattern dystrophies are generally associated with a good visual prognosis and are rarely complicated by choroidal neovascularization. A good anti-VEGF therapy response had been reported.
